106 XLD (Diesel) R Reg. Juddering on start up

When the engine is cold and i start the car up it starts to judder and splutter, smoke comes out of the exhaust. When the engine is warm it starts fine. Also the other day when i was driving along the fuel indicator dropped to below empty then before i had chance to do anything it came back up to a quarter.

Any ideas what this could be?
