Lightbulb Accident Proforma - To Keep in glovebox

Hi folks,

i'd thought that just incase of an unfortunate event that you had an accident this form would come in handy...

the reason i thought about it as going through my work (bus driving) bag i come across a small booklet we carry with forms inside for different scenarios. then i thought it would be a good idea to stick a simple accident form in the glovebox of the car along with the other paperwork in there such as insurance cert, MOT Cert and other stuff i cant remember

anyways i hope some people find it usefull

I purposefully don't keep any documents in the car - they might get stolen

Police never ask for them - they know the answers before they even ask plus you can get 7 days to produce anyhow.

i know where your coming from. but if i leave the paperwork in the house the old man (my father) will move it and forget where he put it. so its safer in the car.

generally in my area there isnt much crime, but if you go up the hill its sierra leon

Very useful, it will enable members to actually have the forms ready to record the accident. You can produce the other documents later if you wish, but at least you wont be scratching details on the back of your hand at the scene.
